Elkhorn Logan Valley Public Health Department (ELVPHD) will hold the Norfolk Vaccines for Children (VFC) Immunization clinic at 302 W. Phillip Ave. The immunization clinic will be held the 1st, 2nd, and 3rd Wednesdays of each month from 10:00 am to 3:00 pm and the 4th Wednesday of each month from 1:00 pm to 6:00 pm. Appointments are required for the Norfolk immunization clinic. If there are no appointments scheduled a nurse will not be available that day. ELVPHD’s immunization program provides childhood immunizations to VFC eligible children as well as for private pay and most insurances. Immunizations are needed for infants at ages 2 months, 4 months, 6 months, 12-15 months & 18 months. For school-aged children, immunizations are needed prior to entry into kindergarten (ages 4-6 years) and prior to entry into 7th grade (11-13 years).
Each child should be accompanied by a parent or legal guardian. A current copy of the child’s past immunization records are required at each visit. A proxy form is needed when a parent or legal guardian is unable to attend the clinic with the child.
