We want you to LOVE YOUR HEART by participating in our heart health education program. ELVPHD's Love Your Heart education classes focus on: blood pressure, nutrition, physical activity, medication management, stress and more! You are welcome to attend the whole series (or attend just a few sessions if you are not able to attend them all). Please see the flyer below which details our Love Your Heart hypertension program and click here to self-enroll in the program and we will contact you to get started. 

Healthcare Providers: We welcome your referrals of patients for our Love Your Heart Hypertension series. Please refer your patients by completing the Healthcare Provider Blood Pressure Self-Managment Class  Referral Form. We will reach out to your patient when the next series is available in their town/area.
