Diabetes was the ranked as one of the highest rated health concerns among adults in our service area according to our most recent community health assessment survey. In addition, 81.7% of survey participants indicated that they felt they needed to lose weight. Because weight loss, nutrition and physical activity levels play an important role in preventing diabetes and many other chronic conditions, ELVPHD is committed to provide weight management and diabetes prevention services throughout our communities.

Diabetes Prevention Program (Scale Down):
Scale Down Weight Loss Program: This is a year-long program and is based upon the National Diabetes Prevention Program. Making lifelong, sustainable healthy lifestyle changes is the goal of the program. We offer the SCALE DOWN program twice a year either virtually or in-person. What can I expect from the SCALE DOWN program?

Weight loss and a healthier lifestyle.

Helps cut your risk of developing Type 2 diabetes in half.

Encourages you to lose 5-7% of your body weight (10-14 pounds for a 200-pound person).

16 weekly core sessions and 6 post-core sessions (once a month)

Provides strategies to incorporate healthier eating habits and physical activity into a person's everyday life.
