Diabetes was the ranked as one of the highest rated health concerns among adults in our service area according to our most recent community health assessment survey. In addition, 81.7% of survey participants indicated that they felt they needed to lose weight. Because weight loss, nutrition and physical activity levels play an important role in preventing diabetes and many other chronic conditions, ELVPHD is committed to provide weight management and diabetes prevention services throughout our communities.
Diabetes Prevention Program (Scale Down):
ELVPHD provides the Diabetes Prevention Program (Scale Down) program. We offer our Scale Down program twice a year in different communities throughout our four-county district. Our program has helped over 200 people lose weight while making lifestyle change that help reduce risk for developing diabetes. Scale Down includes 22 sessions over the course of one year to help people not only lose weight and make healthy lifestyle changes but also to help them learn how to maintain their healthy changes! What can I expect from the SCALE DOWN program?

Weight loss and a healthier lifestyle.

Helps cut your risk of developing Type 2 diabetes in half.

Encourages you to lose 5-7% of your body weight (10-14 pounds for a 200-pound person).

16 weekly core sessions and 6 post-core sessions (once a month)

Our free health coaching program is designed to assist you with knowing your numbers and making healthy changes. This program can be done in-person at any of our three offices (Norfolk, Wisner or Tekamah) or virtually. Our trained health coaches have helped hundreds of people in our service area kickstart healthy lifestyle changes through this program.
