T-Shirt Orders & Donations: Smile in Style Dental Program
Welcome to our Smile in Style program t-shirt fundraiser! Please see the order form below to order your t-shirt. Proceeds from the fundraiser will go to our Smile in Style program which provides fluoride varnish, dental sealants, and education in area schools and childcare facilities. Our services are donation-based, and no child is denied services due to inability to donate.
Thanks to generous program donations, during our past program year we were able to provide dental services to over 1600 children! Each t-shirt purchased will allow us to provide dental services to two children. You can also choose to sponsor a child(ren) for dental services without making a t-shirt purchase.
T-shirts will be available approximately 3 weeks after the purchase date - we will reach out by email when they are ready. Please indicate on the order form if you would like to pick up your t-shirt at one of our office locations (Norfolk, Wisner, or Tekamah) OR if you would like your shirt shipped (for an additional $5 fee).
