Elkhorn Logan Valley Public Health Department has been awarded funding from the Nebraska Department of Health and Human Services in the amount of 15,000 to improve pre-diabetic education efforts throughout Burt, Cuming, Madison and Stanton Counties. The focus of the program is to work with area clinics, hospitals, and other medical professionals to increase the number of pre-diabetic patients referred to evidenced-based diabetes prevention program (DPP) community classes. The classes are based upon the CDC’s National Diabetes Prevention Program and are held throughout the service area by trained instructors.
The program will also work with a Diabetes Prevention Advisory Council (DPAC), which consists of diabetes educators, registered dieticians, and other medical professionals to provide input on diabetes prevention activities and assist with creating an effective referral process and network.
